1. Services Provided
Preventi AI provides preventive cardiology coordination services that may include intake and cardiovascular risk screening, imaging coordination, AI-supported plaque analysis (when applicable), cardiologist consultation and reporting, and prevention guidance. Services vary by package and availability.
2. Not Emergency Care
Preventi AI does not provide emergency services. If you believe you are experiencing a medical emergency, call emergency services immediately.
3. Medical Disclaimer
The Services are intended for preventive education and clinical review support. Preventi AI does not guarantee medical outcomes. All medical decisions should be made in consultation with your treating clinician(s).
4. Eligibility
You must be at least 18 years old to use the Services.
5. Your Responsibilities
You agree to provide accurate and complete information, follow preparation instructions, maintain confidentiality of login credentials, and use the Services lawfully and responsibly.
6. Scheduling & Readiness
Scheduling depends on availability, readiness requirements, and imaging partner policies.
Additional information may be required before services proceed.
7. Payments & Fees
Prices are displayed at checkout and may change over time. Fees reserve clinical and administrative capacity for your selected service. Payment terms are disclosed at the time of purchase.
8. Cancellations & No-Shows
Missed appointments, incomplete intake, or late cancellations may limit rescheduling options and may be subject to imaging partner policies.
9. Communications Consent
By using the Services, you consent to receive operational communications (email, SMS, and phone) related to your care coordination and reporting. You may opt out of marketing communications at any time. Consult messaging, if included, may be time-limited.
10. Acceptable Use
You agree not to attempt unauthorized system access, disrupt platform operations, upload malicious content, or use Services for unlawful purposes.
11. Intellectual Property
All platform content, trademarks, reports (excluding your personal health report), and materials are owned by Preventi AI or its licensors. You may use your personal report for individual use and sharing with your physician.
12. Third-Party Services
Certain Services involve third-party imaging centers, payment processors, or AI vendors. Those third parties may operate under separate policies. Preventi AI is not responsible for systems outside its control.
13. Limitation of Liability
To the maximum extent permitted by law, Services are provided ‘as is’ and ‘as available.’ Preventi AI is not liable for indirect, incidental, or consequential damages. Liability, where permitted, is limited to the amount paid for the specific Service giving rise to a claim.
14. Governing Law
These Terms are governed by the laws of the State of Florida, unless otherwise required by
applicable law.
15. Changes to Terms
We may update these Terms periodically. Continued use of Services constitutes acceptance of revised Terms.
